Assembles the accepted M4 fleet-health design into one durable document. It covers evidence and state precedence, strict lost-boundary repair, typed inbox migration, lead routing, capacity, optional human notification, config, and all five delivery units.

Validation: mvn clean install (672 tests, 0 failures, 0 errors); all three Mermaid diagrams rendered with mermaid-cli.

No Java or wiki files changed.
