Wires control-link, missing-target, readiness-grace, and stall facts into FleetHealthMonitor. The working stall threshold now uses health.workingSuspectAfterSeconds with its 600-second default and 300-second floor.

Tests: mvn -f fleetd/pom.xml clean install (955 tests, BUILD SUCCESS).
